About
Malka Pill, LCSW — Online Therapist for Adults & Older Adults
Licensed Clinical Social Worker specializing in adults and geriatrics, providing telehealth therapy across New York State.

Hi, I'm Malka.
I live in Nassau County, New York with my husband and our three children. Between family, work, and everything in between, I know how full life can get — and I believe therapy should fit into your life rather than add one more pressure to it.
I've spent years working face to face with adults and seniors all around New York City: in homes, in facilities, and in offices, alongside people navigating anxiety, depression, OCD, grief, caregiving, and the quiet weight of getting older. That work taught me that the people who need support most are often the ones for whom getting to an office is hardest.
So I've brought that same in-person care to telehealth. Now I can sit with clients across New York State from wherever they feel most comfortable — a favorite chair, a quiet kitchen, a room where they already feel safe.
My approach is warm, collaborative, and practical. I use CBT skills alongside other strength-based approaches to help you build real tools, understand your own patterns, and feel steadier day to day.
Whether you're moving through a hard season, managing something you've carried for years, or caring for a parent while trying to hold everything else together — you're welcome here.
Why I became a therapist
I was drawn to social work because of the people who get overlooked — the older woman whose grief gets called "expected," the adult who has been quietly anxious since childhood and assumed that was just their personality, the daughter running between her own kids and her father's doctors.
Early in my career I sat with people in their living rooms and at their bedsides. You learn quickly that what helps isn't clever technique — it's someone steady who listens carefully, takes you seriously, and helps you find the next workable step. That's what I try to be in every session.
Moving to telehealth wasn't about stepping back from that work. It was about reaching the people who couldn't get to me: clients with mobility limits, caregivers who can't leave the house for an hour, and anyone for whom the trip itself was the barrier.

My Philosophy
I believe healing happens through relationship. My goal is to create a space where you feel safe enough to be honest, supported enough to take risks, and understood enough to grow. Therapy is not about fixing you, because you are not broken. It is about helping you understand yourself, build skills, and move toward the life you want.
Practically, that means we look at how thoughts, feelings, and habits feed each other, and we work on the ones you can actually change. We also start from what's already working — the resilience, relationships, humor, and faith you've been leaning on long before we met.
Families matter here, too. So much of what we carry is tied to the people we love — partners, children, aging parents — and that belongs in the room with us.
Who I work best with
- Adults who look fine on the outside and are exhausted on the inside
- Older adults facing retirement, loss, health changes, or loneliness
- Adult children caring for aging parents
- People with OCD or intrusive thoughts who are tired of white-knuckling it
- Anyone in a life transition that has knocked them off balance
The heart of my practice is online therapy for seniors and older adults in New York.
See everyone I help →What a first session feels like
Mostly, we talk. I'll ask what brought you here, what's been hardest lately, and some background about your health, your family, and your history. You share as much or as little as you want.
There's nothing to prepare and no right way to say it. Plenty of people start with "I don't really know where to begin" — that's a fine place to begin.
By the end we'll name one or two things to focus on first, and decide together on a rhythm that fits your life.
